Notice of Privacy Practices
Important information for visitors, patients, and families.
Your information · Your rights · Our responsibilities
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Effective date: October 25, 2026.
This Notice of Privacy Practices describes how Paramount Home Health Services may use and disclose protected health information (PHI) in connection with its skilled home health services, and the rights you have under applicable privacy laws. It concerns patient health information, not merely data collected through this website. For the website itself, see our Website Privacy Policy & Terms of Use.
1. Your health information rights
Get an electronic or paper copy of your records
You may request access to, or a copy of, health information maintained in a designated record set. We generally respond within 30 days of receiving your request, subject to applicable extensions and exceptions. We may charge only a permitted, reasonable, cost-based fee.
Ask us to correct your information
You may request an amendment if you believe information in your records is incorrect or incomplete. We may deny a request where permitted by law, but will explain the reason in writing, generally within 60 days.
Ask for confidential communications
You may request that we contact you in a certain way or at another address. We will accommodate reasonable requests.
Request limits on uses or disclosures
You may ask us to restrict certain uses or disclosures for treatment, payment, or health care operations. We are not always required to agree. If you pay in full out of pocket for a service or item, you may ask us not to disclose information about that service or item to your health plan for payment or health care operations, unless disclosure is required by law.
Request an accounting of certain disclosures
You may request a list of certain disclosures of your information made during the six years before your request, subject to legal exceptions. One accounting in a 12-month period is generally free; a permitted cost-based fee may apply for additional accountings.
Get a copy of this notice
You may request a paper copy even if you have agreed to receive it electronically.
Use a personal representative
A person legally authorized to act for you, such as a guardian or an individual with an applicable health care power of attorney, may exercise your rights as permitted by law. We may verify that authority.
File a complaint without retaliation
You may complain to us or to the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.
2. Your choices
Depending on the situation, you may tell us your preferences about sharing relevant information with family, friends, or others involved in your care or payment for care, and in disaster-relief situations. If you cannot express a preference, we may use professional judgment when permitted by law and in your best interest.
We generally need your written authorization for uses and disclosures not otherwise permitted or required by law, including most marketing uses, the sale of PHI, and most disclosures of psychotherapy notes if we maintain any. You may revoke an authorization in writing, except to the extent we have already relied on it.
If we conduct fundraising communications, applicable opt-out rights apply. Special rules apply to any information protected by 42 CFR Part 2, as explained below.
3. How we may use and disclose your information
Treatment
We may use or share health information to provide and coordinate skilled home health services and communicate with other authorized professionals involved in your care. Example: We share relevant care information with a treating clinician to coordinate services.
Payment
We may use or disclose information to bill for services, determine coverage, and obtain payment from applicable payers. Example: We submit information necessary for a payer to review a claim. This does not represent a statement that Paramount participates in any particular insurance plan.
Health care operations
We may use and share information as permitted to manage the agency, support quality reviews, coordinate administrative activities, evaluate services, and meet operational or compliance responsibilities. Example: We review records for authorized quality-assurance activities.
Other uses and disclosures allowed or required by law
- Public health activities and certain health or safety concerns, such as legally required reporting;
- Reports of suspected abuse or neglect when required or permitted by law;
- Health oversight, audits, and investigations authorized by law;
- Research when permitted and subject to applicable conditions;
- Workers’ compensation and specified law-enforcement or government requests;
- Organ and tissue donation requests and disclosures to coroners, medical examiners, or funeral directors when lawful;
- Court orders, legal proceedings, or subpoenas when applicable privacy conditions are met; and
- Other disclosures required by federal or California law.
Some categories of information receive stronger protections under California law, including the Confidentiality of Medical Information Act, and other applicable laws. We will follow the more protective rule when required.
4. Additional protections for substance use disorder records
To the extent we receive, create, or maintain substance use disorder patient records that are protected by 42 CFR Part 2, additional confidentiality requirements and patient rights may apply. Such records generally cannot be used or disclosed in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or an appropriate Part 2 court order accompanied by a subpoena or similar legal mandate, as applicable.
Part 2 information is subject to specific consent, redisclosure, and fundraising requirements. We will honor those requirements when they apply. This statement does not mean Paramount operates a substance use disorder treatment program.
5. Our privacy responsibilities

- We must follow the terms of the notice currently in effect.
- We will notify affected individuals following a breach of unsecured PHI as required by applicable law.
