Getting Started
What is skilled home health care?
Skilled home health brings appropriate nursing, therapy, and related health services into a patient’s home under an individualized care plan. It is different from hospice care and from standalone nonmedical companion or housekeeping services. Explore Paramount’s home health services.
Who may need home health services?
Home health may be appropriate for someone who needs skilled services at home following an illness, injury, hospitalization, or change in health. Eligibility depends on the individual’s clinical needs, the plan of care, and any applicable payer requirements. Ask your healthcare provider and Paramount’s team about the next steps.
Is a healthcare provider’s order needed?
Skilled home health services are generally coordinated through a healthcare provider’s orders and an individualized plan of care. The specific documentation and eligibility requirements depend on the situation and any applicable coverage rules. Paramount can explain the appropriate next step when you contact our office.
How do I begin arranging care with Paramount?
Call Paramount at (818) 818-6573 during office hours to discuss service availability and the process for coordinating with your healthcare provider. Ask our team how referrals and medical information should be shared securely. Do not include protected health information in an ordinary website contact form or general email.
How quickly can services begin?
Start-of-care timing depends on the clinical situation, required orders, service availability, and other arrangements. We cannot promise a specific number of hours before a visit. Please contact our office for current availability and guidance.
Services & Care
Which home health services does Paramount offer?
Paramount’s listed services include Skilled Nursing, Therapy Services, Home Health Aide Services, Medical Social Services, Registered Dietitian Services, and coordination of appropriate supplies and durable medical equipment. The specific services available to a patient depend on clinical needs, the care plan, and current capacity. See our Services page.
How is my home health care plan developed?
The home health care team evaluates relevant needs and coordinates services with the ordering healthcare professional according to the plan of care. Care goals, services, and visit frequency are individualized; they should not be assumed from a general website description.
Can my family or caregiver participate?
Patients may involve family members or caregivers in discussions, care education, and coordination as appropriate to their wishes, consent, and privacy preferences. Let your care team know whom you would like involved.
Will the same clinician visit every time?
Staff assignments can vary based on the services needed and scheduling availability. You may share continuity preferences with the office, but a particular clinician or visit pattern cannot be guaranteed.
Are home health aide services the same as nonmedical home care?
No. A home health aide’s support is delivered within the patient’s home health plan of care and coordinated with the clinical team. It should not be assumed to include unlimited housekeeping, companion care, or independent nonmedical caregiving. Learn about Home Health Aide Services.
Patient Rights & Privacy
Can I participate in decisions about my care?
Patients should be informed about their care and given the opportunity to ask questions, discuss goals, and participate in decisions. If an aspect of your plan is unclear or you have concerns about a proposed service, speak with your treating healthcare professional or home health care team.
What if I have a concern or complaint?
Contact Paramount’s office at (818) 818-6573 during business hours to explain your concern and ask how it will be reviewed. If you need information about a formal complaint process or patient-rights procedures, request the agency’s current written policy.
How does Paramount protect medical information?
Personal and health information should be handled through appropriate privacy and security procedures. Please use the secure communication method provided by the agency for patient records or referrals; ordinary website contact forms and general email are not appropriate channels for sending protected health information. Request the agency’s current privacy notice for detailed practices.
How can I verify a person who says they work for Paramount?
If you are unsure of a visitor’s identity or an appointment, contact the Paramount office through the published telephone number before allowing access to your home. Ask the agency directly about its current staff-identification and scheduling procedures.
Can I request help communicating in another language?
Tell the office what language or communication assistance you need. Paramount can explain what options may be available or how an appropriate arrangement might be made; specific interpreter or multilingual staff availability should be confirmed rather than assumed.
Coverage & Payment
Which counties are in Paramount’s Geographic Service Area?
Paramount’s confirmed Geographic Service Area includes selected communities within Los Angeles, Orange, San Bernardino, Riverside, and Ventura counties. Coverage varies by specific address and available services. View the Geographic Service Area and contact the office to confirm your location.
Does Paramount accept Medicare, Medi-Cal, or private insurance?
Plan participation, provider networks, eligibility, and benefits must be verified for your particular situation. This website does not guarantee that any specific insurance plan is accepted or that a service is covered. Contact Paramount and your insurer to confirm current participation and benefit details.
How much will home health care cost?
Costs depend on the services involved, coverage rules, deductibles or other cost-sharing, and individual eligibility. Do not assume that care is free or fully covered. Ask the agency and your insurance plan for details applicable to your situation.
Are medical supplies or durable medical equipment included?
Paramount can help coordinate appropriate supplies and durable medical equipment with suppliers when needed under the care plan. Product availability, suppliers, coverage, delivery timing, and costs require individual confirmation. Learn about Supplies & DME Coordination.
What does Joint Commission accreditation mean?
The Joint Commission’s Gold Seal of Approval® recognizes organizations evaluated against applicable accreditation standards. Read Paramount’s Joint Commission Accreditation page for more information about what the designation represents.
